You will authorize California Headache and Pain Ctr. (CHPCI) to send text messages to your cell phone to notify of upcoming appointments and other important information related to future appointments ONLY.  You understand that standard text messaging rates will apply to any messages received from CHPCI.

You also understand that you can revoke this permission at any time, by opting out or calling the office.  You agree not to hold CHPCI liable for any electronic messaging charges or fees generated by this service.  You further agree that in the event your cell phone number and or cell provider changes, you will inform CHPCI promptly.

information in a timely manner regarding their appointments.  Their information will not be sold, distributed, or in any other way shared with entities or affiliates outside of this facility.
